Title Identification of CircRNA signature associated with tumor immune infiltration to predict therapeutic efficacy of immunotherapy

Description Circular RNAs (circRNAs) are involved in the characteristics of the tumor microenvironment and the regulation of cancer. However, the clinical implications and regulatory networks of circRNAs in cancer patients undergoing immune checkpoint blockades (ICB) treatment have not been fully elucidated. Here, we applied four well-established circRNA identification tools to characterize the circRNA expression profiles of two independent cohorts of 157 ICB-treated patients with advanced melanoma. We observed overall overexpression of circRNAs in ICB non-responders as compared with responders in both pre-treatment biopsies (PRE) and early during therapy (EDT). We then constructed circRNA-miRNA-mRNA regulatory networks and revealed circRNA-related signaling pathways in the context of ICB treatment. Furthermore, we employed a machine learning approach to identify a novel ICB-related circRNA signature (ICBcircSig) based on progression-free survival-related circRNAs. This ICBcircSig was able to accurately predict the therapeutic efficacy of ICB in three independent patient cohorts and outperforms predictions made based on the expression of PD-L1 and the 20 transcriptome-based features. Overall, our study characterized circRNA profiles and regulatory networks in ICB-treated patients and highlighted the clinical utility of circRNAs as predictive biomarkers of immunotherapy.
