The discovery of extracellular RNAs (exRNAs) in biofluids has sparked considerable interest in their biological functions and clinical applications. Current identification methods, limited in their ability to discern a fraction of the diverse exRNAs, have hindered progress in this field. To comprehensively investigate the exRNAs profile, we employed traditional RNA-seq and enzymatic treatment RNA-seq across diverse biofluids, including human serum, ascites, urine, milk, seminal plasma, and serum from mice, rats, rabbits, and bovines. Our analysis unveiled a distinctive exRNAs landscape predominantly composed of rsRNAs using the enzymatic treatment RNA-seq, diverging from the traditional RNA-seq approach, which predominantly emphasizes miRNAs as the main constituent. Subsequently, employing enzymatic treatment RNA-seq on serum samples from 69 healthy individuals and 51 cancer patients, we demonstrated that serum-derived exRNAs, primarily rsRNAs, achieving a remarkable 94.1% sensitivity and 100% specificity in cancer detection, utilizing sophisticated machine-learning classification algorithms. Building on this foundation, we further characterized tumor-type-specific exRNAs in serum, enabling accurate classification of tumors with unknown primary origins. These results will expand our understanding of the underexplored exRNA species and catalyze further advancements in exRNA biomarker research and holds promise for future clinical applications.
