描述信息 We focused on predicting the efficacy of bevacizumab plus chemotherapy unresectable colorectal cancer liver metastases with radiomics and pathomics, and explored the tumor microenvironment related to radiomic analysis based on the transcriptome sequencing of 14 matched samples obtained from before and after treatment. Our biological interpretation of the radiomic model correlated extracellular matrix remodeling and cancer-associated fibroblasts with poor bevacizumab efficacy. Pathological validation further identified fibroblasts and peritumoral angiogenesis as key resistance drivers. This integrative and comparative omics analysis proposed a new analytical template for uncovering the environmental heterogeneity and treatment resistance of colorectal cancer liver metastases, providing practical theoretical basis for the accurate stratification and translational perspectives for pre-treatment decision-making in the management of colorectal cancer liver metastases.
