We employed single-cell-resolution spatial multi-omics to comprehensively characterize the stromal architecture and immune microenvironment of HCC. Our study analyzed 135 tissue microarray (TMA) cores from 45 patients, spanning tumor regions, invasive margins, and adjacent normal tissue. We applied high-throughput CosMx spatial transcriptomics with both 1,000- and 6,000-gene panels, NanoString DSP spatial proteomics, and histopathological analysis via H&E staining. We also further integrated publicly available scRNA-seq datasets from immunotherapy-treated cohorts. By dissecting the synergistic effects of diverse cell types in the fibrotic niches at single-cell and spatial resolution, our study provides spatial mechanistic insights into how the endothelial-CAF-immune axis mediates HCC progression and the resistance to immunotherapy and targeted therapy, offering potential biomarkers for patient stratification and therapeutic targeting. |
