Head and neck squamous cell carcinoma (HNSCC) exhibits a marked sex disparity in incidence, with a higher prevalence in males than in females. Recent studies indicate that this difference persists even after accounting for smoking and alcohol consumption, underscoring the need to elucidate the underlying biological mechanisms. In this study, we demonstrate that sex differences in HNSCC are androgen dependent and identify androgen receptor (AR) signaling as a key regulator of the tumor immune microenvironment through modulation of CD8+ T cell differentiation and function. Mechanistically, early growth response 4 (EGR4) acts as a direct downstream transcriptional effector of AR and induces CD8+ T cell dysfunction. Clinically, androgen deprivation therapy (ADT) is shown to be an effective therapeutic strategy in HNSCC, suppressing tumor growth in mice while enhancing intratumoral CD8+ T cell function. Furthermore, combination therapy with ADT and immune checkpoint inhibitors results in improved antitumor efficacy. Collectively, these findings identify ADT as a promising approach to enhance the antitumor activity of sex-biased CD8+ T cells in HNSCC and provide a rationale for the development of sex-biased immunotherapies for HNSCC patients. |
