Description | Chronic Obstructive Pulmonary Disease (COPD) is characterized by acute exacerbations (AECOPD), yet reliable diagnostic and prognostic markers for AECOPD remain elusive. Consensus clustering identified two novel AECOPD endotypes: cilium-enriched and cilium-depleted. The cilium module was associated with sIgA and immunoglobulin variable regions across all AECOPD patients, while the cilium-enriched endotype showed a strong association with MUC5AC, indicating an enhanced airway mucosal immune response and increased ciliogenesis to improve impaired mucociliary clearance. In contrast, the cilium-depleted endotype, characterized by lower MUC5AC and decreased immunoglobulin levels, appeared to prioritize actin polymerization and wound healing over ciliogenesis to restore airway integrity. These findings suggest potential biomarkers and novel endotypes that could inform targeted therapies for AECOPD. |
