There is little consensus on how to quantitatively assess immune reconstitution after hematopoietic stem cell transplantation (HSCT) in real time as part of standard of care. We compiled 11150 post-transplant immune profiles of 1945 patients who underwent HSCT between 2012 and 2020 -termed the 'Systems Kinetics of Immune Reconstitution Tianjin' (SKIRT) dataset - with the median and 75th-percentile follow-up durations of 1.3 and 2.1 years, respectively, for immune profiling. We observed high levels of inter-personal variability, described the nonlinear dynamics of immune reconstitution, and characterized how the dynamics was jointly influenced by patient sex, patient age, stem cell source, transplant type, graft-versus-host disease, infection, donor age, and the donor's immune status. We also identified a high-risk immune signature that was predictive of earlier mortality and validated it in two independent sub-cohorts. A web tool, the 'SKIRT Calculator', was constructed to assist clinicians in evaluating the donor's immune status and the patient's progress in immune reconstitution. |
